Reverse Shoulder Glenoid Loosening Test Method: an analysis of fixation between two different offset glenospheres

چکیده
Introduction Recent successes achieved with reverse shoulder arthroplasty (RSA) have led to an expansion of its indications and an increase in the number of commercially available designs in the global marketplace. Despite these development efforts, little guidance exists regarding reverse shoulder test methods and little published data exists regarding reverse shoulder performance standards. At least one of the bench studies published using RSA was performed under idealized bone conditions (e.g. polyurethane bone substitute densities of 30 lb/ft) or were based upon physiologic-irrelevant loading conditions (such as, applying the ASTM 2028 glenoid rocking test which translates a convex humeral head against a concave glenoid to induce edge loading and facilitate the rocking horse mechanism).

HYPOTHESIS The success of shoulder arthroplasty surgery has been limited by a common complication: glenoid implant loosening. Eccentric loading of the glenoid due to migration of the humeral head is considered to be the major cause of glenoid loosening and is referred to as the rocking-horse phenomenon. Glenoid implant loosening may cause pain, limitation of function, and the need for complicat...
متن کاملRetrospective study of scapular notches in reverse shoulder arthroplasties.
We conducted a retrospective study of 36 patients with reverse shoulder prostheses to assess whether placement of the glenoid component affected development of scapular notches. Minimum follow-up was 2 years. Glenosphere position and its relation to scapular notching was analyzed radiographically. The glenosphere overhung the inferior glenoid rim in 19 cases (52.8%) and was flush with the rim i...
متن کاملEffects of acquired glenoid bone defects on surgical technique and clinical outcomes in reverse shoulder arthroplasty.
BACKGROUND Reverse total shoulder arthroplasty is the accepted method of treatment for selected shoulder disorders. The purpose of this study was to compare primary reverse shoulder arthroplasty surgical techniques as well as clinical and radiographic outcomes in patients with acquired glenoid bone defects and in those with normal glenoid morphology. METHODS Preoperative three-dimensional com...
متن کاملRevision of Anatomic Total Shoulder Arthroplasty to Hemiarthroplasty: Does it work?
Background: The projected increase in revision shoulder arthroplasty has increased interest in the outcomes of theseprocedures. Glenoid component removal and conversion to a hemiarthroplasty (HA) is an option for aseptic glenoidloosening after anatomic total shoulder arthroplasty (aTSA).Methods: We identified patients who had undergone revision shoulder arthroplasty over a 15-...
متن کاملHip-inspired implant for revision of failed reverse shoulder arthroplasty with severe glenoid bone loss
BACKGROUND AND PURPOSE Glenoid reconstruction and inverted glenoid re-implantation is strongly advocated in revisions of failed reverse shoulder arthroplasty (RSA). Nevertheless, severe glenoid deficiency may preclude glenoid reconstruction and may dictate less favorable solutions, such as conversion to hemiarthropasty or resection arthropasty. The CAD/CAM shoulder (Stanmore Implants, Elstree, ...
